Requirements for :

(I) Cancellation of Philippine passport

  1. A compact, consigned letter of Application
  2. Affidavit Form
  3. Passport along with the photocopy
  4. Copy of marriage certificate
  5. Copy of citizenship of husband
  6. Copy of passport of the husband
  7. Cancellation charge including the courier charge Ktm - Delhi - Ktm :Nrs. 3300.00 (In words, Three thousand Three hundred only)

(II)  Renewal of Philippine Passport

  1. Application addressed to Consul General
  2. Application Form to be duly filled.
  3. Expired passport along with the copy to be submitted.
  4. Latest photograph size : 1.77' X 1.37" (3 in numbers)
  5. Certification Letter from concerned Organisation is required if the person is working.
  6. Fee & Courier Charge NC Rs 5465 to be deposited at Standard Chartered Bank, New Baneshwor, Account No: 01-0209171-01 Account Name: Consulate of the Republic of Philippines

(III)  Lost Passport

  1. A compact, consigned Letter of Application with detail description and event along with the date and place is to be submitted addressing to the Consul General.
  2. Each and every query of the Application form is to be filled.
  3. Report submitted to the Police against the lost passport has to be authenticate and certify by the concerned branch of the Police station.
  4. Three latest passport size photographs.
  5. Passport fee including the courier charge from Kathmandu-Delhi-Kathmandu NC Rs 8665 to be deposited at Standard Chartered Bank, New Baneshwor, Account No: 01-0209171-01 Account Name: Consulate of the Republic of Philippines

(IV) New passport for minors.

  1. Each and every query of the application form is to be filled
  2. Two latest photographs size : 1.77' X 1.37".
  3. Certified two copies of the birth certificate.(All Original Certificate to be brought for verifying )
  4. Certified two copies of the parents marriage contract.
  5. The pertinent pages of the parents passport certified as the true copies by the consulate.
  6. Passport fee including the courier charge from Ktm - Delhi- Ktm : Nrs. 5465

  1. Affidavit form is available at the Consulate Office.
  2. 2-3 weeks is required for new passport / renewal of the passport
